  1. Download and read the Simplified Prospectus or Key Features Document (KFD).  Check the box to confirm you have read it. 
    You will need Adobe Acrobat Reader to view these documents.

    As you may need to refer to this information at a later date, you will need to either save or print a copy of this document. You will not be able to proceed without doing this. (The Simplified Prospectus is a 70 page document - 507kb.)
  2. Print the application form you need.  Complete it and sign it.
  3. Return your application form to Fidelity by post, (please include a cheque if applicable).
